Sports Medicine

The objective of the Sports Medicine program is to provide the highest standard of care for the student-athletes. We ensure that your child’s health and safety is our top priority, and we pride ourselves in providing quality care when an unforeseen injury occurs. The Certified Athletic Trainer and Sports Medicine Staff oversees roughly 800 athletes annually. Be assured, we adhere to our scope of practice, and will demonstrate sound judgment and referrals when necessary.

Please be advised that ALL participating student-athletes must complete the following before trying out for or participating in any Walnut High School athletic events:

  • Sports Physical Exam (also known as Pre-Participation Exam)
  • WVUSD Release of Liability and Proof of Insurance Form
  • Online forms on athleticclearance.com and Final Verification Form.
 
The medical examination form must be completed and signed by a CA licensed health care provider, preferably by a MD or DO. You may simply download this form by selecting "Forms" in the right column, or physically picking one up from the front office and Athletic Training Clinic.
